ABSTRACT

Gene expression analyses suggest that more than 1000-2000 genes are expressed predominantly in mouse and human testes. Although functional analyses of hundreds of these genes have been performed, there are still many testis-enriched genes whose functions remain unexplored. Analyzing gene function using knockout (KO) mice is a powerful tool to discern if the gene of interest is essential for sperm formation, function, and male fertility in vivo. In this study, we generated KO mice for 12 testis-enriched genes, 1700057G04Rik, 4921539E11Rik, 4930558C23Rik, Cby2, Ldhal6b, Rasef, Slc25a2, Slc25a41, Smim8, Smim9, Tmem210, and Tomm20l, using the clustered regularly interspaced short palindromic repeats /CRISPR-associated protein 9 (CRISPR/Cas9) system. We designed two gRNAs for each gene to excise almost all the protein-coding regions to ensure that the deletions in these genes result in a null mutation. Mating tests of KO mice reveal that these 12 genes are not essential for male fertility, at least when individually ablated, and not together with other potentially compensatory paralogous genes. Our results could prevent other laboratories from expending duplicative effort generating KO mice, for which no apparent phenotype exists.

Resumen Introducción: El neumomediastino se define como la presencia de aire o gas dentro de los planos fasciales del mediastino. Por lo general, es un fenómeno secundario a perforaciones traumáticas del tracto aerodigestivo. El neumomediastino secundario a una fractura orbitaria es un evento raro. Se asocia a complicaciones potencialmente mortales como el neumotórax, el neumopericardio y la mediastinitis. Objetivo: Describir un caso de neumomediastino secundario a una fractura aislada de piso orbitario y su manejo médico-quirúrgico. Caso clínico: Paciente de sexo femenino de 42 años que sufre traumatismo en regiones facial, cervical y torácica desarrollando secundariamente un enfisema subcutáneo panfacial y un neumomediastino, el cual se resuelve exitosamente. Discusión: El neumomediastino secundario a una fractura aislada de piso orbitario es un evento muy raro. El aire puede descender a lo largo de los espacios fasciales hasta el mediastino. En este sentido, sonarse la nariz es un factor de riesgo para desarrollar esta pa-tología. Conclusión: Ocurrido un trauma maxilofacial puede presentarse enfisemas de espacios profundos de la cabeza, cuello e incluso el mediastino.


Introduction: Pneumomediastinum is defined as the presence of air or gas within the fascial planes of the mediastinum. It is usually a phenomenon secondary to traumatic perforations of the aerodigestive tract. Pneumomediastinum secondary to an orbital fracture is a rare event. And it is related to life-threatening complications such as pneumothorax, pneumopericardium and mediastinitis. Aim: To describe a case of pneumomediastinum secondary to an isolated orbital floor fracture and its medical-surgical management. Clinical case: A 42-year-old female patient who suffers trauma to the facial, cervical and thoracic regions, secondary development of a subcutaneous panfacial emphysema and pneumomediastinum, which resolves successfully. Discussion: Pneumomediastinum following an isolated orbital floor fracture is a very rare event. The air can descend along the fascial spaces to the mediastinum. In this sense, blowing your nose is a risk factor to develop this pathology. Conclusion: After a maxillofacial trauma, emphysema of the deep spaces of the head, neck and even the mediastinum can occur

Resumen La endosonografía endoscópica es una alternativa que sirve como guía para la realización de derivaciones biliodigestivas en los casos en los que la colangiopancreatografía retrógrada endoscópica (CPRE) ha fallado. Se han descrito técnicas como la coledocoduodenostomía o coledocoantrostomía guiadas por ultrasonografía endoscópica (USE). Se describe el caso de una paciente de 72 años con adenocarcinoma de páncreas, compromiso portal y de paredes duodenales, en quien se usó la CPRE para intentar una derivación paliativa y fue fallida, por lo cual se realizó la colocación de un stent metálico guiado por endosonografía endoscópica, con adecuada respuesta clínica al tratamiento. En conclusión, el procedimiento es seguro y la endosonografía es una vía alterna efectiva en los casos de CPRE fallidas para lograr derivaciones biliares en casos de obstrucción de la vía biliar de origen maligno.


Abstract Endoscopic endosonography is an alternative to guide biliodigestive shunting in cases where endoscopic retrograde cholangiopancreatography (ERCP) has failed. Techniques such as choledoduodenostomy or choledochoantrostomy guided by endoscopic ultrasonography have been described. This is the case of a 72-year-old patient with pancreatic adenocarcinoma, portal vein and duodenal wall involvement, in whom ERCP to try a palliative shunt failed. Therefore, a metal stent was placed using endoscopic endosonography, with adequate clinical response to the procedure and treatment. It is concluded that the procedure is safe and that endosonography is an effective alternative in cases of ERCP failure to achieve biliary bypass in cases of malignant biliary obstruction.

ABSTRACT

The disease named COVID-19, caused by the SARS-CoV-2 coronavirus, is currently generating a global pandemic. Vaccine development is no doubt the best long-term immunological approach, but in the current epidemiologic and health emergency there is a need for rapid and effective solutions. Convalescent plasma is the only antibody-based therapy available for COVID-19 patients to date. Equine polyclonal antibodies (EpAbs) put forward a sound alternative. The new generation of processed and purified EpAbs containing highly purified F(ab’)2 fragments demonstrated to be safe and well tolerated. EpAbs are easy to manufacture allowing a fast development and scaling up for a treatment. Based on these ideas, we present a new therapeutic product obtained after immunization of horses with the receptor-binding domain of the viral Spike glycoprotein. Our product shows around 50 times more potency in in vitro seroneutralization assays than the average of convalescent plasma. This result may allow us to test the safety and efficacy of this product in a phase 2/3 clinical trial to be conducted in July 2020 in the metropolitan area of Buenos Aires, Argentina.


La enfermedad denominada COVID-19 es causada por el coronavirus SARS-CoV-2 y es actualmente considerada una pandemia a nivel global. El desarrollo de vacunas es sin duda la mejor estrategia a largo plazo, pero debido a la emergencia sanitaria, existe una necesidad urgente de encontrar soluciones rápidas y efectivas para el tratamiento de la enfermedad. Hasta la fecha, el uso de plasma de convalecientes es la única inmunoterapia disponible para pacientes hospitalizados con COVID-19. El uso de anticuerpos policlonales equinos (EpAbs) es otra alternativa terapéutica interesante. La nueva generación de EpAbs incluyen el procesamiento y purificación de los mismos y la obtención de fragmentos F(ab’)2 con alta pureza y un excelente perfil de seguridad en humanos. Los EpAbs son fáciles de producir, lo cual permite el desarrollo rápido y la elaboración a gran escala de un producto terapéutico. En este trabajo mostramos el desarrollo de un suero terapéutico obtenido luego de la inmunización de caballos utilizando el receptor-binding domain de la glicoproteína Spike del virus. Nuestro producto mostró ser alrededor de 50 veces más potente en ensayos de seroneutralización in vitro que el promedio de los plasmas de convalecientes. Estos resultados nos permitirían testear la seguridad y eficacia de nuestro producto en ensayos clínicos de fase 2/3 a realizarse a partir de julio de 2020 en la zona metropolitana de Buenos Aires, Argentina.

ABSTRACT Objective Primary and secondary spine tumors occur frequently and treating them surgically has gained prominence. The objective of this study was to evaluate the outcomes and complications in a series of patients with primary and secondary vertebral tumors treated by en bloc vertebral resection. Methods A retrospective review of the patients who underwent en bloc vertebral resection between 2004 and 2017 was conducted. Results During the study period, 16 patients underwent en bloc vertebral resection. They were divided into primary tumor and metastatic tumor groups, 11 being primary tumors and 5 metastatic tumors. The average follow-up period was 95 months (3-156 months). The patients were treated by the same surgical team and the analysis was carried out by an independent observer. Conclusion In this case series, the results where comparable to those reported in the literature in terms of complications, survival, recurrence and neurological status. It is a demanding surgical technique, with a high percentage of morbidity and mortality, nevertheless, it allows local control and recurrence of the lesion. Level of evidence I; Case Series.


RESUMO Objetivo Os tumores primários e secundários da coluna vertebral ocorrem com frequência e o respectivo tratamento cirúrgico tem tido destaque. O objetivo do presente estudo consistia em avaliar os resultados e complicações em um grupo de pacientes com tumores primários e secundários da coluna vertebral tratados através da ressecção vertebral em bloco. Métodos Uma revisão retrospectiva dos pacientes submetidos à ressecção vertebral em bloco foi realizada entre 2004 e 2017. Resultados Durante o período do estudo, 16 pacientes foram submetidos à ressecção vertebral em bloco. Eles foram divididos em grupos de tumores primários e tumores metastáticos; 11 eram tumores primários e cinco tumores metastáticos. O período médio de acompanhamento foi de 95 meses (três-156 meses). Os pacientes foram tratados pela mesma equipe cirúrgica e a análise foi realizada por um observador independente. Conclusão Nessa série de casos, os resultados foram comparáveis aos relatados na literatura em termos de complicações, sobrevida, recidiva e estado neurológico. É uma técnica cirúrgica exigente, com alto percentual de morbimortalidade, porém permite controle local e recorrência da lesão. Nível de evidência I; Série de Casos.


RESUMEN Objetivo Los tumores primarios y secundarios en columna vertebral son una entidad frecuente. En el tratamiento de los mismos la cirugía ha ganado protagonismo. El objetivo de este trabajo fue evaluar los resultados y complicaciones en una serie de pacientes con tumores vertebrales primarios y secundarios tratados mediante resección vertebral en bloque. Métodos Se realizó una revisión retrospectiva de los pacientes intervenidos mediante resección vertebral en bloque desde el año 2004 hasta el año 2017. Resultados Durante el período evaluado, 16 pacientes fueron intervenidos mediante resección vertebral en bloque. Se los dividió en grupo de tumores primarios y tumores metastásicos, 11 fueron tumores primarios y 5 metastásicos. El tiempo de seguimiento promedio fue de 95 meses (3-156 meses). Los pacientes fueron intervenidos por el mismo equipo quirúrgico y el análisis fue realizado por un observador independiente. Conclusión En la serie, se encontraron resultados comparables a los reportados en la literatura en cuanto a complicaciones, sobrevida, recidiva y estatus neurológico. Es una técnica quirúrgica exigente, con un alto porcentaje de morbilidad y mortalidad, sin embargo, permite el control local y la recurrencia de la lesión. Nivel de evidencia IV; Serie de Casos.

ABSTRACT

Neurological diseases are responsible for approximately 6.8 million deaths every year. They affect up to 1 billion people worldwide and cause significant disability and reduced quality of life. In most neurological disorders, the diagnosis can be challenging; it frequently requires long-term investigation. Thus, the discovery of better diagnostic methods to help in the accurate and fast diagnosis of neurological disorders is crucial. Circulating nucleic acids (CNAs) are defined as any type of DNA or RNA that is present in body biofluids. They can be found within extracellular vesicles or as cell-free DNA and RNA. Currently, CNAs are being explored as potential biomarkers for diseases because they can be obtained using non-invasive methods and may reflect unique characteristics of the biological processes involved in several diseases. CNAs can be especially useful as biomarkers for conditions that involve organs or structures that are difficult to assess, such as the central nervous system. This review presents a critical assessment of the most current literature about the use of plasma and serum CNAs as biomarkers for several aspects of neurological disorders: defining a diagnosis, establishing a prognosis, and monitoring the disease progression and response to therapy. We explored the biological origin, types, and general mechanisms involved in the generation of CNAs in physiological and pathological processes, with specific attention to neurological disorders. In addition, we present some of the future applications of CNAs as non-invasive biomarkers for these diseases.

ABSTRACT

Eighty percent of hepatocarcinomas are inoperable at the moment of diagnosis. Liver transplantation is the treatment of choice in these cases, but local therapies are another alternative. Among these, Image-Guided BrachyAblation is a safe choice. We report a 76-year-old male with a hepatocarcinoma, who was considered inoperable due to the high surgical risk of the patient. A local treatment with Image-Guided BrachyAblation was decided. A brachytherapy needle was placed in the tumor under computed tomography guidance and a 15 Gy single dose was delivered from an Iridium-192 source. The patient had no immediate complications and at one month of follow up he continued without incidents.

Introducción: Las fracturas de platillo tibial representan el 1% de todas las fracturas. El resultado funcional depende principalmente del rango de movilidad de la rodilla, la estabilidad de la articulación y el dolor. El objetivo de este estudio fue evaluar el resultado funcional, el rango de movilidad, las complicaciones posoperatorias y el dolor en pacientes que sufrieron fracturas de platillo tibial por traumatismos de alta energía. Materiales y Métodos: Serie de 51 pacientes (edad promedio 48 años), con un seguimiento promedio de 42 meses. Se evaluaron el rango de movilidad, los resultados funcionales con el test de KOOS y el dolor con la escala analógica visual. Resultados: Veinticinco pacientes tenían fracturas Schatzker tipo V y 26, tipo VI. El rango de movilidad promedio era de 2,4°-120,68° en el grupo de fracturas tipo V y de 1,5°-118,04° en el otro grupo. Respecto del dolor, hubo diferencias significativas (p = 0,0403) en pacientes con fracturas Schatzker tipos V y VI: el 50% de los pacientes con fracturas tipo V no refirió dolor y el 50% de aquellos con fractura Schatzker tipo VI indicó dolor >2 y un 25%, <3. Conclusiones: Las fracturas de platillo tibial siguen siendo un cuadro en el que los resultados modifican considerablemente la calidad de vida de los pacientes. Por ello, se les debe informar sobre la posibilidad de malos resultados funcionales a largo plazo y múltiples cirugías; como así también que es posible que no recuperen el nivel previo de actividad deportiva y laboral. Introduction: Tibial plateau fractures represent 1% of all fractures. Functional results mainly depend on the range of motion, joint stability and pain. The objective of this study was to evaluate functional results, range of motion, postoperative complications and pain in patients with tibial plateau fractures due to high-energy trauma. Methods: Series of 51 patients (average age, 48 years) with an average follow-up of 42 months. Range of motion, functional results using KOOS scale and pain with visual analogue scale were evaluated. Results: A total of 25 patients had Schatzker type V fractures and 26 suffered type VI fractures. Average range of motion was 2.4º-120.68º in patients with type V fractures, and 1.5º-118.04º in the other group. As regards pain, significant differences were found (p=0.0403) in patients with types V and VI fractures: 50% of those with type V fractures did not report pain, and 50% of those with type VI fractures indicated pain >2 and 25% referred pain >3. Conclusions: Tibial plateau fractures are a condition where results modify considerably the quality of life. Therefore, patients should be informed about the possibility of bad long-term functional results and multiple surgeries. They must also be warned that it is possible that previous level of sports and work activity will be possibly not achieved. Level of Evidence: IV

ABSTRACT

Introducción: Las múltiples causas de Rabdomiolisis se pueden separar en 5 categorías según el mecanismo de daño del miocito: Hipóxicas, físicas, químicas, biológicas e idiopáticas. Como primera causa se describe el consumo de drogas ilícitas/alcohol, seguido por medicamentos, trauma, inmovilidad y ejercicio extenuante. Las causas químicas representan hasta el 80% de los casos de rabdomiolisis. En esta categoría se encuentran los medicamentos, alcohol y drogas donde destaca la cocaína. Se presenta el caso de un paciente masculino que cursa con insuficiencia renal aguda secundario a una rabdomiolisis por consumo de cocaína.


Introduction: The multiple causes of rhabdomyolysis can be separated into 5 categories according to the myocyte damage mechanism: hypoxic, physical, chemical and biological. However, a cause will not always be identified. The consumption of illicit drugs / alcohol has been identified as the first cause, followed by medication, trauma, immobility and extenuating exercise. The chemical causes represent up to 80% of cases of rhabdomyolysis. This category includes medications, alcohol and drugs, where cocaine stands out. Cocaine is a strong stimulant used mainly as a recreational drug that has considerably increased its consumption in the country so it should be considered. Next, we present the case of a male patient with acute renal failure secondary to rhabdomyolysis due to cocaine use. The physiopathology, diagnostic process and current management will be discussed according to the latest guidelines.

INTRODUCCIÓN: El dolor que se asocia al trabajo de parto (TDP) afecta a todas las mujeres y puede producir alteraciones tanto maternas como fetales, e incluso interferir con el desarrollo normal del proceso. OBJETIVO: Conocer el grado de cumplimiento de solicitud de analgesia peridural en partos vaginales en el servicio de preparto del Hospital Hernán Henríquez Aravena (HHHA) versus la analgesia administrada. MATERIAL Y MÉTODO: Estudio descriptivo retrospectivo, realizado en base a datos del sistema perinatal del servicio de Ginecología y Obstetricia del HHHA, periodo 2014-2016. RESULTADOS: Del total de partos Vaginales entre los años 2014-2016 solo se solicitó Analgesia Peridural en 56.5% de ellos. De las analgesias solicitadas en éste período se administraron un 98%. CONCLUSIÓN: Basado en los registros clínicos, el grado de cumplimiento es cercano al 100% en las analgesias solicitadas. Se observó un incremento anual entre 2014 y 2016 de solicitud de anestesia en procedimientos de parto, y a pesar de esto la eficiencia del hospital no se ha visto afectada. No obstante, se espera que el porcentaje de solicitudes siga en aumento manteniendo el alto nivel de eficiencia. Es pertinente plantear la realización de estudios para extrapolar este resultado a nivel regional y nacional.


INTRODUCTION: Pain associated with labor affects all women and can cause both maternal and fetal alterations and even interfere with the normal development of the process. Objective: to know the degree of compliance with the request for epidural analgesia in vaginal deliveries versus the analgesia administered at the Hernán Henríquez Aravena Hospital (HHHA). Method: Retrospective descriptive study, based on data from the perinatal system of the HHHA Gynecology and Obstetrics Service, period 2014-2016. Results: Of the total number of Vaginal births between 2014-2016 only 56.5% of them were requested for epidural analgesia. Of the analgesia requested in this period, 98% were administered. Conclusion: Based on the clinical records, the degree of compliance is close to 100% in the requested analgesia. There was an annual increase between 2014 and 2016 in the application of anesthesia in childbirth procedures, and despite this, hospital efficiency has not been affected. However, the percentage of applications is expected to continue to increase while maintaining the high level of efficiency. It is pertinent to propose studies to extrapolate this result at regional and national levels.

ABSTRACT

Introducción: La pérdida de hueso proximal en la cirugía de revisión del vástago femoral es el principal desafío que enfrenta el cirujano al efectuar una revisión. El objetivo es obtener una fijación estable de los componentes, así como la restauración de la cinemática de la articulación. Materiales y Métodos: Evaluamos a 37 pacientes (39 revisiones femorales), entre 2010 y 2014, a quienes se les colocaron tallos cónicos no cementados de fijación distal (28 modulares y 11 no modulares). La edad promedio fue 63.5 años. Se incluyó a 18 (48,64%) mujeres y 19 (51,35%) hombres. Se intervinieron 14 (35,89%) casos de aflojamiento aséptico, 14 (35,89%) infecciones, 7 (17,94%) fracturas periprotésicas, 3 (7,69%) fracturas de vástago y una (2,56%) por inestabilidad. El seguimiento medio fue de 42 meses (rango 24-74). Resultados: El puntaje medio de Harris mejoró de 37 (rango 10-77) antes de la operación a 81 (rango 33-96) en el último seguimiento. Cuatro pacientes (10,2%) presentaron luxación, uno (2,5%) tuvo una infección profunda; otro (2,5%), una fractura intraoperatoria; en tres (7,6%) pacientes, se produjo un hundimiento y fue necesario revisar 2 (5,1%) implantes femorales. Conclusiones: Con el vástago cónico se han logrado resultados satisfactorios en 2-6 años de seguimiento, en varias condiciones de revisión femoral. Requiere de una técnica simple y reproducible, con una mejoría clínica ampliamente publicada. Nivel de Evidencia: IV


Introduction: Proximal bone loss at the femoral stem is the main challenge facing surgeons at the time of revision total hip arthroplasty. The aim of revision surgeries is to obtain a stable placement of components and to restore joint kinematics. Methods: Between 2010 and 2014, we evaluated 37 patients (39 hips) in whom non-cemented femoral stems were used (28 modular and 11 non-modular).The average age was 63.5 years. Study patients included 18 (48.64%) women and 19 (51.35%) men. Causes of reoperation included 14 (35.89%) cases of aseptic loosening, 14 (35.89%) infections, 7 (17.94%) periprosthetic fractures, 3 (7.69%) stem fractures and one (2.56%) for instability. Mean follow-up was 42 months (range 24-74). Results: The average Harris score improved from 37 (range 10-77) points before surgery to 81 points (range 33-96) at the last follow-up. Four patients (10.2%) presented dislocation, one (2.5%) suffered a deep infection, one (2.5%) had an intra-operative fracture; 3 (7.6%) patients presented subsidence and it was necessary to revise 2 (5.1%) femoral implants. Conclusions: Satisfactory results were obtained with the femoral stem in 2 to 6 years of follow-up in several conditions of femoral revision surgeries. It requires a simple and reproducible technique with clinical improvement that has been widely published in the literature. Level of Evidence: IV

ABSTRACT

Resumen La perforación duodenal posterior a la colangiopancreatografía retrógrada endoscópica (CPRE) es una complicación infrecuente que sucede en un 0,1%-0,6% de los casos. El manejo (quirúrgico o no quirúrgico) depende de varios factores. Presentamos el caso de una mujer que sufrió una perforación duodenal post-CPRE manejada conservadoramente con un stent biliar metálico autoexpandible (SMAE) totalmente recubierto y antibióticos, quien no requirió manejo quirúrgico.


Abstract Post-ERCP duodenal perforations occur in only 0.1 to 0.6% of ERCP cases. Whether these occurrences are managed with or without surgery depends on several factors. We report the case of a woman who had a post-ERCP duodenal perforation that was conservatively managed with a fully covered self-expanding metal stent (FCSEMS) and antibiotics who did not require surgical management.

ABSTRACT

La literatura reporta una creciente incorporación y participación de la psicología comunitaria en políticas públicas, que genera obstáculos, tensiones y posibilidades. Cobra relevancia académica y profesional analizar los antecedentes sobre transferencia de conocimiento desde la psicología comunitaria hacia las políticas públicas. Se revisa y expone literatura respecto de las condiciones que afectan el vínculo entre la producción académica y las respuestas a los problemas sociales desde la perspectiva de las deliberative politcs, del proceso de encuentro y de la lucha de interpretaciones de actores como factor de cambio de las políticas públicas. Desde la psicología comunitaria, se analiza la complejidad de transferencia del conocimiento hacia las políticas públicas, concluyendo con recomendaciones para potenciar la contribución y comprender las tensiones reportadas.


The literature reports the growing incorporation and participation of community psychology on public policies, and considering the obstacles and tensions in this relationship as reported by literature,-it is both academically and professionally relevant to review the knowledge transfer from the academia to the policy making process. We review a body of literature regarding the link between academic production and the response to social problems, emphasizing the deliberative politics approach, with a description of stakeholders'confluent and conflicting interpretations as a factor of change for public policies. We also review the literature regarding the complexity of knowledge transfer from community psychology to public policy, as reported by this discipline, concluding with recommendations for enhancing its contribution and to understand the tensions reported.

ABSTRACT

In South America, yellow fever (YF) is an established infectious disease that has been identified outside of its traditional endemic areas, affecting human and nonhuman primate (NHP) populations. In the epidemics that occurred in Argentina between 2007-2009, several outbreaks affecting humans and howler monkeys (Alouatta spp) were reported, highlighting the importance of this disease in the context of conservation medicine and public health policies. Considering the lack of information about YF dynamics in New World NHP, our main goal was to apply modelling tools to better understand YF transmission dynamics among endangered brown howler monkey (Alouatta guariba clamitans) populations in northeastern Argentina. Two complementary modelling tools were used to evaluate brown howler population dynamics in the presence of the disease: Vortex, a stochastic demographic simulation model, and Outbreak, a stochastic disease epidemiology simulation. The baseline model of YF disease epidemiology predicted a very high probability of population decline over the next 100 years. We believe the modelling approach discussed here is a reasonable description of the disease and its effects on the howler monkey population and can be useful to support evidence-based decision-making to guide actions at a regional level.

En este trabajo se informa el hallazgo de huevos de Diphyllobothrium sp. en ejemplares de Canis familiaris (perro doméstico) de Puerto Iguazú, una ciudad subtropical de la provincia de Misiones, Argentina. Durante 2013, en el marco de un relevamiento de la fauna parasitológica de los perros de Puerto Iguazú, se detectaron dos casos positivos en la búsqueda de huevos de Diphyllobothrium sp. La materia fecal de los perros fue recolectada en frascos con formol al 10% y procesada mediante las técnicas de sedimentación de Telemann y de flotación de Sheather. Dado que Misiones no forma parte de la zona endémica de difilobotriasis y considerando, además, su ubicación fronteriza, este hallazgo reviste gran importancia para la salud pública. Se señala la necesidad de actualizar el estado de conocimiento sobre el ciclo de vida de estos parásitos identificando el rango de hospederos intermediarios y definitivos, su potencial zoonótico y la situación epidemiológica en áreas no endémicas


This paper reports the first finding of Diphyllobothrium sp. eggs in Canis familiaris (domestic dog) from Puerto Iguazú, a subtropical city of Misiones province, Argentina. In 2013, two positive cases of Diphyllobothrium sp. eggs were detected during an annual parasitological survey of dogs. Dog feces were collected in vials containing 10% formalin and processed using Telemann's sedimentation and Sheather's flotation techniques. The two cases were detected in rural areas of the municipality. Since Misiones is not a part of the endemic area of diphyllobothriasis and given the fact that it is located in the three-border area of Argentina, Brazil and Paraguay, we consider this finding of great importance to public health. We stress the need for updating the current knowledge about the life cycle of these parasites considering the range of intermediate and definitive hosts, their zoonotic potential, and the epidemiological situation in non-endemic areas

ABSTRACT

Introducción: La secuencia Moebius se caracteriza por el compromiso congénito de los nervios motor ocular externo y facial, y se puede asociar al compromiso de otros pares craneales y a otros defectos congénitos. Su etiología es multifactorial y no bien definida, actualmente la teoría más aceptada es la disrupción vascular durante el desarrollo del romboencéfalo. Su incidencia exacta se desconoce, pero impresiona estar en aumento y asociada a la exposición prenatal a teratógenos. Objetivos: Analizar las historias clínicas de 30 pacientes con secuencia Moebius y las características de su compromiso ocular. Materiales y métodos: Estudio retrospectivo, transversal, observacional y descriptivo. Se analizaron 30 historias clínicas de pacientes con secuencia Moebius atendidos por vez primera entre el mes de Julio de 1999 y Junio de 2012 por el servicio de Oftalmología del Hospital Garrahan. Resultados: Se estudiaron 30 pacientes 15 de sexo femenino y 15 de sexo masculino, dentro de los antecedentes maternos 7 madres refirieron ingesta de misoprostol y 4 tuvieron metrorragias durante el primer trimestre de embarazo. Todos los pacientes tuvieron compromiso del VII nervio; en 20 pacientes fue bilateral y simétrico; y en los restantes asimétrico. Todos los pacientes tuvieron compromiso del VI nervio bilateral, a algunos de los cuáles se les efectuó cirugía de estrabismo otros están en plan de cirugía y unos pocos no la requirieron por presentar fijación de ambos ojos en posición primaria de la mirada. Conclusión: la secuencia Moebius es una rara patología genética y congénita multifactorial y de compromiso multisistémico que ha visto incrementada su frecuencia desde el uso de ciertos fármacos teratógenos y que obliga a una intervención quirúrgica precoz de neuroortopedistas, oftalmólogos, cirujanos plásticos y control clínico multidisciplinario para brindarles a estos niños las mejores posibilidades de desarrollo funcional y estético reparador (AU)


Introduction: Moebius syndrome is characterized by congenital palsy of the external and facial oculomotpr nerves, and may be associated with involvement of other cranial nerves and congenital defects. The etiology is multifactorial and not well defined. Currently, the most widely accepted theory is a rhombencephalic maldevelopment. The true incidence of Moebius syndrome is unknown, but it seems to be increasing associated with prenatal exposure to teratogenic factors. Objectives: To analyze the clinical charts of 30 patients with Moebius syndrome assessing ocular involvement. Material and methods: A retrospective, cross-sectional, observational study. Thirty clinical charts of patients with Moebius syndrome that were first seen at the Department of Ophthalmology of Hospital Garrahan between July 1999 and June 2012 were assessed. Results: Of the 30 patients 15 were female and 15 male. Maternal history showed seven mothers that received misoprostol and four that had metrorrhagia in the first trimester of pregnancy. All patients had VII cranial nerve involvement; the involvement was bilateral and symmetric in 20 and asymmetric in the remaining patients. All patients had bilateral VI nerve involvement, some of whom underwent surgery for strabismus, others are on the list for surgery, and a few do not require surgery because of fixation of both eyes in primary gaze position. Conclusion: Moebius syndrome is a rare multifactorial genetic and congenital pathology with multisystemic involvement and increased incidence because of the use of teratogenic drugs requiring early surgical intervention by neuroorthopedic and plastic surgeons, and ophthalmologists, and a multidisciplinary follow-up to provide these children with the best possibilities for functional development and aesthetic repair (AU)

ABSTRACT

Aims: The aim of this paper was to assess the promptness of caregiver’s action at the onset of malaria symptoms to giving recommended effective anti-malaria drugs from public health facility. Study Design: It was a cross sectional study. Study Setting: The study was conducted in Akpabuyo, a rural community in Cross River State, south eastern Nigeria, March to May 2011. Methods: Announcements were made in the community inviting mothers/caregivers to bring their sick-children/wards to the public primary health facilities for free laboratory malaria test and treatment. Caregivers treatment seeking behaviour were assessed using an interviewer administered questionnaire. Data was collected from each of the ten electoral wards that make up Akpabuyo local government area (LGA) in Cross River State, Nigeria. Wherever necessary native language or broken-English was used. Results: Eight hundred and sixty children between the ages of 6 months to 14 years participated in the study. Majority of them (68%) were under-five years of age. There were slightly more males (52%) than females, mean age of the children was 47 months. The mean time-lag between onset of symptom and presentation in the public health-care was about 168 hours. The mothers either bought their drugs from patent medicine vendors and/or used herbal-medicines, before eventually going to the public health facility, where there are both free drugs and skilled manpower. Twenty-seven (27%) of the caregivers did not give any known treatment. Conclusion: Mothers/caregivers in this community treat their malaria sick children within 168hours, after lots of delay, trying other remedies, before presenting in public health facilities. There is need to improve their treatment seeking behaviour, using health education intervention and behavioural change communication.

Fusobacterium spp. son bacterias anaerobias Gram negativas. La osteomielitis vertebral por dichas bacterias es muy rara, de hecho, sólo podemos encontrar 11 casos en la literatura. Se presenta un caso de un varón de 46 años con dolor lumbar irradiado a la pierna derecha, de varias semanas de evolución y que no respondió al tratamiento con AINEs. Para el diagnóstico se utiliza la RMN, una biopsia con drenaje de la colección y una PCR universal seguida de secuenciación de ADNr 16S, con la que se obtuvo el diagnóstico microbiológico del paciente, identificando un Fusobacterium nucleatum como responsable. Posteriormente se pautó clindamicina como tratamiento final. En conclusión, la espondilodiscitis por Fusobacterium spp. es una entidad rara y su diagnóstico es a menudo difícil, tanto por las características clínicas como por la dificultad de obtener el diagnóstico microbiológico apropiado. La biopsia vertebral y las técnicas moleculares microbiológicas como la PCR ADNr Universal, son esenciales para la identificación del organismo y permiten la determinación de un diagnóstico y un tratamiento antibiótico apropiados.


Fusobacterium spp. are Gram negative anaerobe bacteria. Vertebral osteomyelitis caused by these bacteria is very unusual; in fact, we could only find 11 cases in the literature. We report the case of a male, 46 year-old patient who had had lumbar pain for several weeks that irradiated to the right leg, and did not respond to NSAID treatment. The work-up included MRI, biopsy with draining of the collection and a universal PCR followed by 16S rDNA sequencing. The latter was used to make the microbiologic diagnosis, which identified Fusobacterium nucleatum as the causative agent. Final treatment consisted of clindamycin. In conclusion, spondylodiscitis due to Fusobacterium spp. is a rare and difficult to diagnose entity, due both to its clinical characteristics and to the difficulty in making the right microbiologic diagnosis. Vertebral biopsy and molecular microbiologic techniques such as Universal PCR rDNa, are essential to identifying the organism, making the diagnosis and prescribing appropriate antibiotic therapy.

ABSTRACT

Se presenta el caso de una paciente de seis años, de sexo femenino sin antecedentes mórbidos ni familiares de importancia, quien consulta por proptosis izquierda en abril de 2011. Se realiza resonancia magnética de cerebro evidenciándose un tumor de seno etmoidal con compromiso de órbita izquierda que ocasiona exoftalmo. La biopsia endoscópica nasal fue compatible con fibroma osificante juvenil. Posteriormente se realiza antrostomía con etmoidectomía en dos tiempos, con resultados satisfactorios para la paciente. Aunque el fibroma osificante juvenil es un tipo de lesión benigna poco frecuente, puede llegar a ser muy agresiva y con una alta tasa de recidiva en ciertas ocasiones, por lo que se debe tener en cuenta para realizar un diagnóstico y tratamiento precoz, con un seguimiento programado a largo plazo.


This paper presents the case of a patient of six years old female without background or important morbidly in her family, who consults for left-sided proptosis in April of 2011. It performs brain magnetic resonancebeing demonstrated a tumor of theethmoid sinus with commitment to the left orbit that causes exophthalmus. Nasal endoscopic biopsy was compatible with juvenile ossifying fibroma. It was subsequently performed ananthrostomy whitethmoidectomy in two times, with satisfactory results for the patient. Although the Juvenile ossifying fibroma is a rare type of benign lesion, can be very aggressive and have a high recurrence rate in certain occasions, by what should be taken into account to make an early diagnosis and treatment, with a scheduled follow up in the long term.
